Faith Formation Enrollment Information

Enrollment is underway!!!

-Enrollment Information was mailed out on August 1st.  Please watch your mail for information on how to get your family signed up for faith formation this year!!

 

Due Date--August 20th!!! (Firm Date)

You can drop off your forms at the office, in the box in the gathering space, or drop them in the collection baskets at Mass.

Where you can find all of the forms:

       --In the parish gathering space

Forms that need to be filled out:

  1. Completed Enrollment Form (Must fill out one of the two options for this form)
    1. On-line option with Google forms--https://goo.gl/forms/zHxTzL7vGK4z7brU2 (You need to submit one of these forms for each child)
    2. Print and manual option (On website to the right of this)
  2. Completed Medical Form B (Fillable Form B on Website to the right of this)
  3. Completed Medical Form C (fillable Form C on Website to the right of this)
  4. Signed Child Release Form (On Website to the right of this)
  5. Tuition & Fees Letter
    1. There are no fees if you are a member of the parish and have a filled out Stewardship Form for this year.
    2. Stewardship/ Parish Registration Form are available at the office if you need to complete yours for this year      
  6. Second Grade - Copy of Baptismal Certificate (If not baptized at St. Nicholas)
  7. Eighth Grade - Copies of Baptismal & First Eucharist Certificates (If not done at St. Nicholas)

Faith Formation at St. Nicholas

“Catechesis is an education in the faith of children, young people, and adults which includes especially the teaching of Christian doctrine imparted… with a view to initiating the hearers into the fullness of Christian life.” (Catechism of the Catholic Church, #5)

Realizing that the aim of catechesis “is to lead believers to a deeper knowledge and love of Christ and the Church and a firm commitment to follow him.,” (National Catechetical Directory) the goal of the faith formation process is to assist parents in the religious formation of their children by providing religious formation sessions for students Age 4 through Grade 12.
